- Create a NEW Patient Agreement in MotionMD
- Add the following information...
-Patient's Name & DOB
-Provider's name
-Diagnosis Code
- Product
-Payment Method
- Attach the Paper Patient Agreement image via "Add" in the Attachment section along with the demographics
- In the Patient Signature box, choose 'Attach AOB'.
- Select 'Attach Script' in the Provider Signature Section
- Submit the PA to billing
